Adult D SAR

A Safeguarding Adult Review has now been published.
Today, the LSAB publish a Safeguarding Adult Review (SAR) in relation to Adult D.
The findings from the review found that it was unlikely that any of the agencies supporting Adult D could have prevented this death, however some learning has been highlighted for agencies to strengthen their procedures to allow earlier identification of the issues raised and ensure they are addressed more effectively.
The SAR Sub Group are working to address the key learning and recommendations made and a learning brief has been produced to support agencies in recognising and implementing improvements. Good practice was also highlighted during the review, which is reflected in the report and learning brief.
